Guided Self Placement Student Information Survey
Definition of Self-Assessment Survey – A process or a tool used to encourage students to reflect on their academic history and educational goals that may include a self-evaluation on the familiarity and comfort with topics in English or mathematics. Survey results may culminate in course recommendations. This survey may be part of the college’s student onboarding process.
This brief questionnaire was developed and vetted with faculty and students at the ASCCC Guided Pathways Regional Meetings during fall 2019. The question set is very long, but the goal was to provide a comprehensive list of questions for colleges to consider and customize, selecting or modifying questions that are relevant to the college's processes with language familiar to college culture and institutional nomenclature. Students are asked to bring the completed form to their counseling appointment related to enrolling in courses, exploring an education goal or developing a student education plan or when speaking with a faculty advisor.
